From this page you could access every online birth database for the state of Arizona, such as Arizona birth certificates, birth records, birth indexes, Arizona birth notices, and other related records. 

The Arizona Department of Health Services, the Vital Records Office contains records for births that occurred in the state from the late 1800s. 

Please note that only the following individuals can order a certified copy of a birth certificate: 

  • The registrant
  • Wife or husband of the registrant
  • Parents, foster parents, guardian or grandparents of the registrant
  • Adult brother, sister or child of the registrant
  • Attorney representing the registrant
  • Other individuals or agencies allowed by law 

Please mail your completed application along with a photocopy of your valid picture identification and appropriate fee to the Office of Vital Records with the following address:

From the date your request is received by the office, the processing time for applications is usually between 15 to 20 business days.
