The Arkansas Department of Health has records of births dating from February 1, 1914 to the present. Prior to 1914, only a limited number of records are on file.
Only the following individuals can order a certified copy of a birth certificate:
- The person named on the record
- The parent or child of the person
- The spouse of the person
Needed information:
- Full name at birth (if known, please give original name, if the child has been adopted)
- Date of birth
- Sex
- Race
- Age/last birthday
- Place of birth
- Name of hospital
- Name of attendant at birth
- Full maiden name of mother
- Full name of father
- Your reason for requesting this certified copy
- Your relationship to the child
- Address, telephone number and signature and of the applicant
Send your order including application form, proper fee ($12 per certificate copy) and a copy of your legal valid ID to the Vital Records Section located at:

You can also request a certified copy by going to the office addressed above in person in the opening hours from 8:00 a.m. to 4:00 p.m. Monday through Friday.
