The Colorado Department of Public Health, The Office of Vital Records preserves legal certificates of vital events in the state. You may need certified copies of these official documents for many purposes including applying for a passport, qualifying for insurance benefits, establishing proof of age, establishing proof of citizenship, and etc.
Only following individuals can order a certified copy of a birth certificate:
- The individual named on the record (the registrant)
- The parents, stepparents, grandparents or great grandparents of the person
- The spouse or siblings of the person
- Adult children, stepchildren or grandchildren of the person
- Legal guardian of the person
- A legal representative of above individuals
Mail your order along with completed application form, needed fee ($17.75 per copy) and a photocopy of your valid photo ID to the office with the address below:
