From this page you could access every online birth database for the state of Florida, such as Florida birth certificates, birth records, birth notices, Florida birth indexes, and other related records. 

Florida Department of Health, Office of Vital Statistics reserves records for birth, death, marriage and other vital events. Now the Office includes about 21.5 million vital records. 

Since the mid 1980s, Florida has been issuing computer certification that is issued on a safety paper, with an embossed seal. The certificate meets requirements established by national and international groups including Homeland Security, and is accepted by the Social Security Administration, the U.S. Passport Office, school districts, courts, and all other federal and state entities requiring proof of U.S. birth. 

You might want certified copies of birth certificate for employment, passports, proof of citizenship school entrance, insurance, governmental benefits, or etc. 

In order to order a birth certificate, you should be one of the following individuals: 

  • The person named on the record (must be 18 or older)
  • The parent, legal representative or guardian of the child
  • A person with a court order 

And you should also provide the information below: 

  • Full name of child at birth
  • City or county of birth
  • Date of birth
  • Father’s full name
  • Mother’s full name including her maiden name
  • Your full name
  • Mailing address (including state, city and zip code)
  • Daytime Phone number
  • Relationship to the individual named on the certificate
  • Signature of applicant 

Send your request including completed application, required fee and a photocopy of a valid picture ID through mail to the Vital Statistics office with the following address:

The fee for a computer generated certification is $9 and it generally will be processed in 10 business days. You can also go in person to the Office located at:
