Submit your birth certificate request by mail
The Illinois state’s records on births, deaths, and other vital records are maintained by The Illinois Department of Public Health. Each year, more than 400,000 records are registered. The Department began to collect birth records in 1916 and has since placed 27 million records on file, as well as some records from the mid 1800s.
Besides the Illinois Department of Public Health, circuit clerks and county clerks might have the birth documents on file. You could obtain the birth records older than 1916 from the county clerk’s office located in the birth place of the person.
Only the following persons are able to order certified birth certificate copies:
- The individual whose name is on the record (if aged 18 or more.)
- The parent(s) of the person.
- A legal representative or legal guardian of the person.
However, genealogical researchers and others could access uncertified copies on a restricted basis.
You need to provide the information below when ordering a birth record:
- The person’s full name at birth.
- Date of birth.
- The name of the parent(s) shown on the record.
- The place where the birth occurred (including city, county and hospital, if known).
- A copy of your valid photo ID card.
You should complete the application and mail it along with the required fee (a check or money order) and other needed documents to the following address:

For a full certified copy you should pay $15 and from the date your request is received, it takes about 3 to 4 weeks to be processed.
You could also go in person to the above address and request a birth certificate at the front window. (8:30 a.m. – 4 p.m. | Monday – Friday, excluding holidays)
