The Kansas Department of Health and Environment, the Vital Statistics Office maintains records for vital events such as births, marriages, divorces, and deaths which happen in the state. Adding around 100,000 new vital records annually, the Office preserves over 10 million records totally and issues more than 360,000 certified copies of these records to qualified applicants each year. 

People need these records to perform daily activities such as enrolling in schools, starting new jobs, sports participation, collecting life insurance benefits, obtaining passports, qualifying for subsidized housing, and transferring property. 

Certified copies of birth certificates can be requested by the following individuals: 

  • The person named on the certificate
  • Direct family members
  • A legal representative
  • A named beneficiary or anyone with a direct interest

Mail your certified copy order including completed form, a copy of a government issued photo ID and required fee ($12 for a copy) to the Office with the following address:
