Adding around 100,000 new vital records annually, the Office preserves over 10 million records totally and issues more than 360,000 certified copies of these records to qualified applicants each year.

Certified copies of birth certificates can be requested by the following individuals:

- Direct family members
- A legal representative

Mail your certified copy order including completed form, a copy of a government issued photo ID and required fee ($12 for a copy) to the Office with the following address:
