The Registry of Vital Records and Statistics reserves marriage, birth and death records that take place in the state of Massachusetts from 1916 to the present.
Since 1635, vital records have been indexed in Massachusetts. And In 1841, statewide collection started. Records for births that happened from 1841-1915 are accessible through the State Archives. The city or town of occurrence may contain earlier records.

- Full name of the person at birth
- City or town of birth
- Date of birth
- Full father’s and mother’s name (including maiden name)
Send your certificate orders including requested information, completed form and required fee to the Massachusetts Registry office located at:

The cost for a certificate copy requested by mail is $28.00. Orders are generally processed within 20-30 business days after the order receipt. Regardless of intended use, the Registry office issues only certified copies of birth certificates.
You could also visit the Registry counter in person during the following hours: 8:45 a.m. – 4:45 p.m. (Monday – Friday, except state holidays)
The fee is $18.00 for a certified copy ordered at the Registry counter.
