The Mississippi State Department of Health, The Office of Vital Records maintains birth, death, marriage, divorce and other records for vital events since November 1912. You should visit the Department of Archives and History for older records.
Only one of the following individuals can order a certified copy of a birth certificate:
- The person named on file
- The parent, grandparent or legal guardian of the person
- The child or grandchild of the person
- The brother or sister of the person
- The spouse of the person
- The legal representative of any of the above persons
Needed information:
- Full name on certificate
- Place of birth (county/city/state)
- Date of birth (month/day/year)
- Sex (female/male)
- Full maiden name of mother
- Full name of father
- Your reason to order a certificate
- Registrant’s relationship to you
- Your mailing address and telephone number (including are code)
- Your signature
You should send the completed application, plus required fee ($15.00 for the first copy) and a photocopy of your valid ID through the mail to the Vital Records Office with the following address:

If no record can be found, you will be issued a “Not-On-File” certification, but the fee for the first copy will not be refunded. However, you will get back additional fees paid for extra certificate copies.
