The Department of Public Health and Human Services, Office of Vital Statistics maintains all records for vital events that happen in the state of Montana.
Only following individuals can order a birth certificate:
- The registrant (must be at least 14 years old)
- The registrant’s parents or grandparents
- The registrant’s children or spouse
- A guardian, caretaker relative or an authorized representative
- Individuals who have a right by law
Required information:
- Full (first, middle and last) name on the record
- Date of birth
- Place (city or county) of birth
- Father’s full name
- Mother’s full maiden name
- Reason the certificate is needed
- Your relationship to the certificate holder
- Daytime phone number
- Mailing address
- Your signature
Mail your completed application, proper fee ($12.00 per certified copy) and a photocopy of your valid photo ID to the office with the following address:
