Nebraska Department of Health & Human Services, Division of Public Health maintains birth, death, marriage, divorce and other vital records for the state of Nebraska. 

A certified copy of a birth certificate can only be issued with a proper reason. You may order a certificate if you are one of the following individuals: 

  • The person named on the record
  • The spouse of the person
  • The parent or child of the person 

Required information: 

  • Child’s full name on certificate
  • County of birth
  • City or town of birth
  • Month/day/year of birth
  • Mother’s full maiden name
  • Father’s full name
  • The reason the record is needed
  • If not your record, you relation to the person
  • A photocopy of the current valid photo id of the applicant
