A certified copy of a birth certificate can only be issued with a proper reason. You may order a certificate if you are one of the following individuals:
- The person named on the record
- The spouse of the person
- The parent or child of the person
Required information:
- Child’s full name on certificate
- County of birth
- City or town of birth
- Month/day/year of birth
- Mother’s full maiden name
- Father’s full name
- The reason the record is needed
- If not your record, you relation to the person
- A photocopy of the current valid photo id of the applicant
