The records for vital events that happen in New Mexico are maintained by the Department of Heath, the Vital Records & Health Statistics Office.
To order a certified copy of a birth certificate, only one of the following persons can apply:
- The registrant
- Father, mother, or maternal/paternal grandparent of the person
- Current husband or wife of the person
- Child of the person
- Brother or sister of the person
Needed information:
- The birth name of the registrant
- The date and city (if known, including the county) of birth
- Full maiden name of mother
- The father’s full name
- Applicant’s relationship to the person
- The reason for requesting the certificate
- Your name, signature and mailing address
You should send your request along with the application form, a photocopy of your valid ID and proper fee ($10 per certified copy) through mail to the office with the following address:
