From this page you could access every New York State online birth database, such as birth records, New York birth certificates, birth indexes, birth notices, and other related records.
The New York State Department of Health, The Office of Vital Records reserves certificates and records for births, deaths, marriages, divorces and other vital events that happen statewide except for New York City whose records are maintained by NYC Department of Health and Mental Hygiene.
The Municipal Archives in Manhattan has on file records of births occurred before 1910 for the five Boroughs of New York City (Manhattan, Queens, Bronx, Brooklyn and Staten Island).
To order a certified copy of a birth certificate you should be:
- The individual named on the record
- A parent of the registrant
- A spouse, child or other persons (Only by an order of a NY Court)
Required information:
- The registrant’s full name as listed on the certificate
- Date of birth
- Town, city or village (and if known, name of hospital) where birth occurred
- Full Father’s name and mother’s maiden name
- The reason for which certificate copy is needed
- The applicant’s relationship to the registrant
- The applicant’s name, phone number, address and signature
You should mail your order for a NY certified copy including application form, proper fee ($30.00 per copy) and a photocopy of a valid ID to the Certification Unit of the office with the following address:

From the day you mailed order is received, it takes within 6-8 weeks to be processed.
You can apply for a NYC birth certificate by going to the Vital Records Office in person. The office is located in Lower Manhattan, at 125 Worth Street. The opening hours are from 9:00 a.m. to 3:30 p.m. Please note that a $40 fee is needed for most corrections to certificates.
