From this page you could access every Ohio state online birth database, such as birth records, Ohio birth certificates, birth notices, Ohio birth indexes, and other vital records.
All the records for vital events that occur in the state are maintained by the Ohio Department of Health, The Office of Vital Statistics.
Needed information to order a certified copy of a birth certificate:
- Name at birth (if adopted, provide adopted info)
- Place of birth (city/county)
- Date of birth
- Father’s name
- Mother’s Maiden name
- Your mailing address to which the certified copy will be mailed
- Your area code and telephone number
Mail your certificate copy request including application form, proper fee ($21.50 for a copy) and a photocopy of your valid photo ID to the office with the following address:

The certified copy delivery time may take between 2-3 weeks after your order is received in the office. You can also use the walk-in service by going in person to the office located at:
