From this page you could access every online birth database for the state of Oregon, such as birth records, Oregon birth certificates, birth notices, Oregon birth indexes, and other vital records.
The Oregon Department of Human Services, The Center for Health Statistics maintains and issues certificates for vital events such as birth, marriage, divorce, and death that happened statewide. The office has records for births dating from July, 1903.
Only following persons could order a certified copy of a birth certificate:
- The individual name on the record
- The person’s immediate family members
- The person’s partner, legal guardians or legal representatives
Needed information:
- Full name of the person named on the certificate
- Date and place (city or county) of birth
- Mother’s full maiden name
- Father’s full name
- Your name and relationship to the person
- Your telephone number and mailing address
- Your signature
You should main your request including completed application form, a photocopy of your official ID and proper fee ($20 per copy) to the office with the following address:

Please wait up to five weeks for your mailed order to be processed and delivered to your address.
You can also go in person to the Office of Vital Records located at 800 NE Oregon Street, Portland. The opening hours are from 8:00 a.m. to 4:50 p.m. Monday through Friday.
