Submit your birth certificate request by mail
To apply for a certified copy of your birth certificate or an immediate family member‘s, you must be at least eighteen years old. Certified copies of birth record with the raised seal are issued by the Pennsylvania Department of Health, the Division of Vital Records that are acceptable for various uses, i.e. employment, social security, passport, and personal identification. The Division of Vital Records keeps Records for births from 1906 to the present. You can access birth records prior to 1906 through the courthouse in the person’s birthplace county.
Persons who are eligible to request a birth certificate:
- The person whose name is on the birth record
- Legal representative
- Immediate family members (wife; husband; son or daughter; parent/step-parent; sister/brother/half sister/half brother/; grandchild/great grandchild; grandparent/great grandparent etc.) If the person named on the certificate is not alive, an individual who is not an “immediate family member” – but still a family member – must send death certificate copy to be qualified to get the birth record.

Required information:
- Full name of the person (at birth) named on the birth certificate (if it has changed since birth for any reason other than marriage like court order, adoption, etc., include changed name)
- Gender of the person
- Date of birth (If you don’t know the year of birth, you may request a multi-year birth records search.)
- City and county of birth
- Parents’ names, including mother’s maiden name
- Reason for the request
- Relationship to the person
- Applicant’s mailing address
- Applicant’s daytime telephone number, including area code
- Applicant’s signature
You must send the required documents (completed application form, self-addressed stamped envelope, a copy of your photo ID and payment) to:
