From this page you could access every Rhode Island online birth database, such as birth records, Rhode Island birth certificates, birth notices, Rhode Island birth indexes, and other related records. 

The Rhode Island Department of Health, the Office of Vital Records is responsible for filing and reserving records for vital events that happen in the state. Certified copies of records may be needed for many reasons, including establishing proof of age, receiving death benefits from life insurance policies, and receiving social security benefits. 

Only following persons can order a certified copy of a birth certificate: 

  • The person named on the file
  • A member of the registrant’s immediate family
  • The registrant’s guardian or an authorized agent of above individuals
  • A person required by law or who has a court order 

Needed information: 

  • The name of the record
  • The date and town/city where the birth occurred
  • Why you need the certificate copy
  • Your relationship to the individual named on the certificate
  • The mother’s maiden name and the name of the father
  • Your mailing address and a telephone number 

Please send your completed application form, correct fee ($20.00 for each certified copy) and a photocopy of your valid photo ID through mail to the Vital Records Office located at:

Please allow between 4-6 weeks until your certificate copy will be mailed to you by the office. 

You can also go in person to the office from 12:30 p.m. to 4 p.m., Monday through Friday.
