From this page you could access every online birth database for the state of Utah, such as birth records, Utah birth certificates, birth indexes, Utah birth notices, and other records.
The Utah Department of Health, The Vital Records Office is in charge of filing all vital events such as birth, death, marriage and divorce which happen in Utah. The office reserves records for births that happened in Utah since 1905.
Please note that only immediate family members (like a spouse, father, mother, grandparent, sibling, child and etc.) or individuals with written permission from them can order a certified copy of a birth certificate.
Needed information:
- Full name of the person on the certificate
- Place/date of birth
- Full maiden name of mother
- Full name of father
- Your relationship to the registrant
- Your mailing address
- Your telephone number
- Your signature
Send the completed application form, a photocopy of the front and back of your ID and required fee ($18.00 per certified copy) to the office with the following address:
