Statewide vital events registration in Vermont started in 1857, and the Department of Health maintains vital statistics and records for vital events that are not older than five years. All other records are accessible through the Archives and Records Administration.
Currently, the Vermont vital records system contains all kinds of vital events such as births, marriages, divorces, deaths, abortions, fetal deaths, and etc.
When a birth happens, the midwife, physician, or other birth attendant (usually the medical records staff for hospital births) has to complete a birth certificate. Then the certificate is filed and recorded in the birth place town, and the Department of Health gets a certified copy of it.
Needed information to order a certified copy of a birth certificate:
- The child’s date of birth
- City or town of birth
- Maiden name of mother
- Name of father
- Your name
- Address
- Phone number
- Intended use of certificate
- Your relationship to the individual on the record:
- Your signature
