The West Virginia Health Statistics Center, The Vital Registration Office is responsible for collecting records of vital events such as birth, death, marriage, and divorce.
The individuals who can order a certified copy of a birth certificate are:

- The husband or wife of the person
- The parents and grandparents of person
- The sisters or brothers of the person
- The children and grandchildren of the person
- Any individual who can prove a legal interest in the certificate
Needed information:
- Name of the person as it appears on the certificate
- Date of birth
- The city and county of birth
- Father’s full name and the mother’s full maiden name
Send your request including completed application, required fee ($12 per certificate copy) and a photocopy of a valid ID to the office addressed below:
