From this page you could access every online birth database for the Wisconsin state, such as birth records, Wisconsin birth certificates, Wisconsin birth indexes, birth notices, and other records. 

Wisconsin Department of Health Services, The Office of Vital Records files and preserves birth, death, marriage, divorce and other records for vital events that occur statewide. 

You can order a certified copy of a birth certificate if you are: 

  • The person named on the certificate
  • The current domestic partner of the person
  • The current wife or husband of the person
  • A parent or grandparent of the person
  • A sibling or child of the person
  • A person authorized by one of the above
  • A person who are authorized by law to request a certificate 

Send your application form, required fee, a photocopy of a valid photo ID and a self-addressed, stamped envelope through the mail to the office at with the following address:

The fee for a record search is $20.00, which includes one copy of the certified copy. Additional copies cost $3.00 each. Your mailed order will be processed within one month. 

You can also apply in person by going to 1 West Wilson Street, in Madison. The opening hours are between 8:00 A.M. and 4:15 P.M., (Monday – Friday, except official holidays.)
