The Wyoming Department of Health, the Office of Vital Statistics Services is responsible for maintaining records for vital events such as birth, marriage, death, divorce that occurred in the state since July 1990. For older records you should visit Wyoming State Archives.

Following persons could order a certified copy of a birth certificate:

  • Registrant (should be at least 18 years old)
  • Parent(s) of the registrant
  • Legal guardian with needed documents
  • Lawyer representing the parent(s) or registrant

Needed information:

  • Full name on the certificate
  • Date of birth
  • County or city of birth
  • Full names of the father and mother including mother’s maiden name
  • Mailing address where certified copy is to be mailed
  • Signature of the applicant

You should mail your request including application form, a photocopy of a valid ID (such as State ID card, the driver’s license, or passport) and required fee ($13 for each certificate copy) to the office located at:
